Data Fields on the Course Proposal Form
Most fields on the course proposal form, revised fall 2005 are identical to
fields on previous course proposal forms. Descriptions for these can be found
elsewhere in the Source Book. Self explanatory fields and questions on Sides B, C, and D are not addressed here. Instructions for all other new or revised fields plus some additional information on old fields are presented below according to their top-down position on the form.

| New Program or Major, if applicable |
If the course proposal is submitted as part of a newly proposed program or major,
list the title of the new program or major here. Otherwise, leave this blank.

| Regularization |
Regularization is not a new option, and must be used if you plan on offering the course
more than twice without formal Senate approval. It is the process of submitting a
Selected Topics course for approval as a regular course with a permanent number, title
and description. Selected Topics courses are subject to the
"Two Semester Rule". Although you may have offered this
course several times under Selected Topics it is considered a new course and the proposal
must be treated as such.
For regularizations complete all of SECTION I and SECTION II
SIDE A of the Course Proposal Form. In SECTION I: COURSE IN PRESENT FORM provide the
most recent Selected Topics department prefix, number, section title, and credits that
were used for the course.
You must also address all of the questions on SIDE B. If the
course is to be offered online follow the directions for proposing an online course and
proceed to fill out sides C and D. Refer to Filling Out the Course Proposal Form

| Contact Minutes per Week |
The New York State Education Department defines a credit hour as 50 minutes of
contact per week for 15 weeks, i.e., 750 minutes/1 credit per semester, 2250 minutes/3
credits per semester. Final exams as well as lectures, recitations, discussions and
labs may be counted toward the total.. And at least 30 hours of supplementary

| Grading Option |
1 = A-F, 3 = Remedial, 6 = P/F, 7 = Not Required,
9 = P-F&V, 2 = A-F&V, A = Audit
(V=variable length). For more

| Frequency of Offering |
Indicate how often the course will be offered: S = every semester,
Y = at least once every academic year, E = offered every other year
when the fall semester occurs in an even year, O = offered every other year
when the fall semester occurs in an odd year, SI = upon sufficient student
interest, IR = irregularly, SS = summer session only. |
| If repeatable, note maximum credits |
If this course can be repeated for credit, indicate the maximum number of credits
or times a student can repeat the course for credit towards a degree.

| Variable Title on Section Level |
The following are the only courses which may have variable titles:1)
Selected Topics (course numbers end in 00), 2) Workshops, 3) Readings and
Research ,4) Seminars, and 5) Various courses which include the following
key phrases in the title: Practicum in..., Problems in..., Themes in..., Topics in...

| Title on Transcript |
Provide the course title as it appears or will appear abbreviated
on a class list, transcript, or PeopleSoft Course Catalog. 30 character limit
including spaces.

| Catalog Description (35 word limit) |
If this is to be changed, provide the existing course description as it appears in
SU's Undergraduate or Graduate Course Catalog.
|
| Prerequisites |
When changing a course description:
- Complete the Preq. field in Section I and II, even if the preq. is to remain unchanged.
- If the department wishes to delete the preq. please state "Delete preq." in the prerequisite field of Section II.
