Course Proposal Form, Revised Spring 2009

Currently, the only version of the Course Proposal Form available from this Web page is in a Microsoft Word document format. Filling out the course proposal form.

In order to make hyperlinks work users of Word 2007 will need to download the document and after opening it proceed to enable macros.

Microsoft Word Document 

Please download the Word document to your computer, complete the form in Word, and then print it.

To download:

1. Right click on proposal form.
2. From the pop-up menu that opens, select Save Link As (or Save Target As).
3. In the Save As dialog box, select the folder where you would like the file saved.

Once the file is downloaded to your computer:

1. Open Windows Explorer.
2. Go to the folder where you saved the file.
3. Double-click the file to open it in Word.
4. Complete the form online in Word and click Save.
5. Print the form.
