Course Proposal Form, Revised fall 2007
Currently, the only version of the Course Proposal Form available from this Web page is in a Microsoft Word document format.
Filling out the course proposal form.
Microsoft Word Document 
Please download the Word document to your computer, complete the form in Word, and then print it.
To download:
- Right click on proposal form.
- From the pop-up menu that opens, select Save Link As (or Save Target As).
- In the Save As dialog box, select the folder where you would like the file saved.
NOTE If you experience difficulties with opening the file after downloading it with Netscape,
please try downloading the file with Internet Explorer.
Once the file is downloaded to your computer:
- Open Windows Explorer.
- Go to the folder where you saved the file.
- Double-click the file to open it in Word.
- Complete the form online in Word and click Save.
- Print the form.
Portable Document Format (PDF) 
Due to technical problems the PDF version of the Course Proposal Form is not available at this time.
